[#1260] Split new tool form and handle with JS for html5 conformance.
Overlapping hierarchies of forms and tables are not standard-conformant.
Either a form should wrap entire table, or a cell should contain entire form.
There're bunch of small forms already, so take new tool form and split into
per-cell input forms, then collect values before submission.
